Safe Stimulant Medication Management

Stimulant medications such as Adderall, Vyvanse, Concerta, Azstarys, and Focalin are classified as Schedule II controlled substances. These medications can be highly effective for treating ADHD when prescribed appropriately, but they require careful evaluation, monitoring, and compliance with state and federal regulations.
At The Healed Mind Psychiatry, we prioritize safety, ethical prescribing, and responsible monitoring.

Before Starting a Stimulant Medication
Stimulant treatment is not initiated without:
• Confirmed ADHD diagnosis
• Comprehensive psychiatric evaluation
• Review of the Texas Prescription Monitoring Program (PDMP)
• Baseline blood pressure and heart rate
• Substance use screening
• Signed informed consent
Stimulant medication is prescribed only when clinically appropriate.

Mandatory Monitoring Requirements
Because stimulants are controlled substances, ongoing monitoring is required.

For Patients Age 16 and Older:
• Urine drug screening is mandatory upon initiation
• Urine drug screening is required every three (3) months thereafter
• PDMP is reviewed routinely
• Periodic in-person visits are required
Failure to comply with monitoring requirements may result in modification or discontinuation of stimulant treatment.

Cardiac Safety Requirements (Age 40 and Older)
For patients age 40 and older:
Stimulants will not be started or continued without:
• An EKG completed within the past 12 months
• Cardiologist clearance when indicated
• Active management by a Primary Care Provider
These requirements are in place to protect cardiovascular health and reduce medical risk.

Prescription Policies
• Prescriptions are typically issued in 30-day increments
• Early refills are not routinely granted
• Lost or stolen medications are not automatically replaced
• Pharmacy stock shortages must be verified
Repeated early refill requests or inconsistent monitoring may lead to reassessment of treatment.

In-Person & Telehealth Visits
We offer both in-person visits in the Denton/Aubrey area and secure telehealth appointments across Texas and New Mexico.
Patients prescribed stimulant medication must attend required in-person visits as determined by clinical judgment and regulatory guidance.
Additional in-person visits may be required if:
• Vital sign abnormalities are present
• Safety concerns arise
• Monitoring requirements are due

When Stimulant Treatment May Be Discontinued
Stimulant prescribing may be discontinued if:
• Medication misuse or diversion is suspected
• Urine drug screen results are inconsistent
• Required monitoring is not completed
• There is threatening or inappropriate behavior toward staff
• Clinical benefit is not demonstrated
